Responsibilities
At patient’s initial appointment take the client’s medical history, evaluate the symptoms and develop a rehabilitation plan that aligns with the client’s personal needs and goals.
This will include:

Teaching the client different functional tasks and exercises intended to strengthen and/or stretch muscles as well as alleviate pain.

Diagnosing movement dysfunction and developing a treatment plan

Teaching patients how to properly use therapeutic exercise techniques

Use of pain-relieving modalities such as electrical stimulation, ultrasound, moist heat/cold applications appropriately.

Assisting patients with the use of equipment such as wheelchairs or walkers

Maintaining patient records and keeping track of goals and progress

Writing progress notes and discharge summaries

Using outcome measures as a means of measurement of client’s progress

Providing home exercise programs
-
Communicating with home care provider methods of assisting the client with transfers and home exercises.
